Do I need an eye appointment to verify whether or not I can undergo laser eye surgery? Do I need a referral?

Depending on which laser centre you choose, you might be able to have the pre-operative assessment at that laser centre, while some request that it be completed at your optometrist's office.  In general, it is best to have a complete assessment with your optometrist in order that your options can be discussed in detail.  Should you decide to proceed with laser surgery, your optometrist will be able to refer you appropriately.
